NEW PATIENTS

BEFORE Your First Appointment:

  1. Set up an account on our electronic portal 

    If you have never accessed our Patient Portal, please contact our office to request that we activate your account and receive your login information.

  2. Check your email for your new Portal ID and Password (You’ll need this at your appointment!)
  3. In the portal, review your contact and insurance information, medications, allergies, past medical history, and skin disease history sections. 
  4. Make sure to SAVE your information in the portal as you complete it.  
  5. Bring any prescription and regular skincare products that you are currently using with you to your visit, and if you are able to come without makeup, please do so.
  6. Arrive 15 minutes before your scheduled appointment time to finalize your electronic access and complete our paperless electronic waivers.

If you have never accessed our Patient Portal, please contact our office to request that we activate your account and receive your login information.

If you cannot complete the electronic access, you may print and fill out the paper version.

CANCELLATION POLICY

Dr. Holcomb strives to see all patients of Pure Dermatology in a timely and efficient manner.  As a courtesy, we attempt to contact each patient 24 to 48 hours prior to their scheduled appointment to remind them of the date and time.  However, it is the responsibility of the patient to arrive on time for their appointment.   

 

Your time is valuable, as is ours, and if you are unable to keep your reservation, we request that you give our office a minimum of 24 HOURS notice if you need to cancel or reschedule an appointment. Failure to do so will result in a missed appointment fee, which is NOT covered by insurance.

 

The missed appointment fees are as follows: 

 

  • Medical – $50.00
  • Surgery / Cosmetic – A minimum of $100.00 for each appointment on the missed day. If you have prepaid for a package and fail to show or cancel your appointment then 1 session will be deducted from your package. Higher fees may be applicable for procedures like Botox, fillers, lasers, and PRP injections.
  • Patch testing for skin allergies – $500.00 (3 visits and special preparation) will require a $500 nonrefundable deposit to book your appointment.  Your $500 will be forfeited if you do not cancel 1 WEEK BEFORE (MONDAY BEFORE) your scheduled appointment.
