Excessive Sweating at Pure Dermatology

PureDerm Medical Dermatology Specialists

Sweating is a natural and necessary bodily function that helps regulate body temperature and removes toxins. When sweating becomes excessive, persistent, or embarrassing, it is  often referred to as “hyperhidrosis.” Hyperhidrosis affects millions globally and can significantly impact one’s quality of life.

What is Hyperhidrosis?

Hyperhidrosis refers to the condition of excessive sweating beyond what the body needs for temperature regulation. It can affect the entire body or be limited to one  or a few body parts. Patients complain of extra sweating on the armpits, hands, feet, face, scalp or even the whole body. People with hyperhidrosis may sweat when it’s hot, when they exercise, or even when they’re relaxed and cool. For some, it can occur at unpredictable times, causing anxiety and discomfort.

There are two main types of hyperhidrosis or Excessive Sweating:

Primary Focal Hyperhidrosis: This type is more localized and typically affects the hands, feet, armpits, face, or scalp. It often starts in childhood or adolescence and isn’t necessarily linked to other medical conditions. Primary Focal Hyperhidrosis tends to run in families, suggesting a genetic link. It is believed to be caused by overactivity in the sweat glands, particularly in areas like the hands, feet, scalp, face, and armpits. Primary Focal Hyperhidrosis can be worsened by stress, anxiety, and emotional triggers. It can also rarely occur on the buttocks or genitals.

Secondary Generalized Hyperhidrosis: This type is caused by an underlying health condition or as a side effect of medication. It tends to affect larger areas of the body and can start later in life. If you are sweating all over, you should see your primary doctor or dermatologist about possible causes like hormonal changes, diabetes, hyperthyroidism, and neurological disorders.

Impact on Daily Life

Excessive sweating can severely disrupt daily activities and social interactions.

Embarrassment and Anxiety: Many people with hyperhidrosis feel self-conscious about their sweating, leading to anxiety in social or professional situations. This can create a vicious cycle, where the stress of sweating leads to even more perspiration.

Physical Discomfort: Wet clothing, slippery hands, and sweaty feet can make tasks like writing, holding objects, or shaking hands uncomfortable.

Skin Problems: Constant moisture can lead to skin irritation, rashes, and fungal infections.

Limited Clothing Choices: People with hyperhidrosis often find themselves restricted in their clothing choices, opting for dark or loose-fitting clothing to hide sweat stains.

Reduced Quality of Life: The emotional and physical toll can affect mental health, leading to low self-esteem, social withdrawal, and even depression.

Treatment Options for Hyperhidrosis

While hyperhidrosis can be frustrating, there are numerous options to effectively treatment and manage excessive sweating.

Antiperspirants

Over-the-counter antiperspirants: Many individuals start with these products, which help block sweat ducts. These often have the word “clinical” in them and may contain aluminum chloride.

Prescription-strength antiperspirants: For more severe cases, your dermatologist may recommend a prescription strength aluminum chloride antiperspirant. These may sometimes irritate the skin and they need to be titrated differently for each patient. They are very safe.

Topical anticholinergics

This is a newer class of prescription medication based on an oral treatment usefor hyperhidrosis. This is currently the brand name Qbrexa and it is a cloth that you wipe on the area that has excess sweating, so it is best used for Primary Focal Hyperhidrosis.  It is only FDA approved for use in the armpits but may dermatologists use it off-label in other areas like the forehead, hands, or feet.

Oral Prescription Medications

Anticholinergics like glycopyrrolate: These medications work by reducing overall sweat production. They can have side effects such as dry mouth, blurred vision, and urinary retention. Because they block sweat throughout the body, they are ideal for people with excessive sweating in more than one area or people with severe sweating. Patients must use caution to make sure to cool themselves periodically or avoid use when they are in a very hot environment.

Beta-blockers like propranolol: These medications are often used to control anxiety-induced and flushing sweating. They work best for those with situational sweating.

In-office treatments

Botox Injections: This is commonly known for its use in cosmetic treatments, but Botox, Dysport, Jeuveau, and Daxxify can also be used to temporarily block the nerves that cause sweating. Injections are typically administered in areas such as the armpits, hands, or feetThis is difficult to get covered by insurance so most patients opt to pay out of pocket for this procedure which can run between $1000-$1200 per treatment. Results can last several months but need to be repeated. Some patients report getting close to a year out of this treatment, especially when combined with other modalities.

At-home treatments

Iontophoresis involves immersing the hands or feet in water while a mild electrical current is passed through. The current helps reduce sweating, and it’s especially useful for people with hyperhidrosis in these areas. The drawback of this procedure it that you must repeat it multiple times in a week.

Lifestyle Changes: Many people adopt changes to deal with their hyperhidrosis. This includes wearing loose, breathable fabrics; avoiding known triggers such as spicy foods, caffeine, and stress; staying prepared by carrying a change of clothing, wipes, and handkerchiefs to freshen up during the day; and practicing mindfulness or relaxation techniques to manage anxiety-related sweating

Surgical Options

Sympathectomy involves cutting or clamping the nerves responsible for excessive sweating of the armpits. While this is considered a permanent option, this is usually considered a last resort due to potential risks and long-term side effects.

Surgical removal of sweat glands is also considered a permanent option for excessive underarm sweating but this can often result in scarring.

Coping Strategies

Living with hyperhidrosis can be challenging, but there are ways to manage its impact on your life:

Talk to a Professional: Consulting a dermatologist or healthcare provider can help you find the best treatment for your specific case.

Support Groups: Connecting with others who experience hyperhidrosis can help alleviate feelings of isolation and provide practical advice. The International Hyperhidrosis Society is a great resource for those who suffer from excessive sweating.

Excessive sweating, while frustrating, doesn’t have to control your life. Understanding the causes and exploring treatment options can provide relief and improve both your physical comfort and emotional well-being. If you or someone you know is struggling with hyperhidrosis, consult with a dermatologist to discuss the best course of action.
